This is a missable achievement. To get this achievement you will have to go to Blim City after the 3rd bounty (Douglas), then go up the street to the right after leaving your house. Here you will find a small NPC called Stan. Talk To Him and select the “I’ll Buy Some Cum” option. Make sure you do this before starting the final level of the game. Then just finish the game with the item he gives you in your inventory to get the achievement during the credits.
